forms: case control on advanced controls example page doesn't work

see ​http://localhost:8888/advanced-controls.html
When selecting 'mail' the address fields are not displayed (and the phone field is not disabled)

The examples in ​http://localhost:8888/case-control.html work.

case control did work on this page, but some other issues arised
